Overview
The Department of Public Health seeks to hire an additional Community Health Worker (CHW) to assist in providing necessary information to Lynn residents and support for ongoing Public Health initiatives include its outreach to senior citizens as the City continues its own recovery and prevention efforts resulting from the COVID-19 pandemic. The CHW will serve under the auspices of the Public Health Department, and report to the Director of Public Health and Director of Elder Services.

Duties and Qualifications
• Employ dynamic communication and interpersonal skills, cultural competency/sensitivity, tactful language, and empathetic skills to build rapport and maintain trust with residents of varied backgrounds.
• Obtain and document relevant information, including contact demographics, underlying medical conditions, and other risk factors.
• Coordinate referrals for testing, healthcare, and other supportive services, as needed, per local protocols.
• Assess resident ability, with adequate water, food, and other necessities. Identify barriers to necessary interventions and facilitate appropriate referrals, per local protocols. Collaborate with key service providers to transfer client information and ensure expedited initiation of appropriate medical or social support services, per local protocols.
• Utilize a computer, tablet, and/or cell phone with appropriate access to required applications, databases, and/or web-based platforms,
• Participate in requisite trainings, regular program meetings and quality monitoring improvement activities in order to ensure and enhance the activities and program outcomes.
• Maintain patient/resident confidentiality.
• Facilitating health checks and tests, as well as referring residents to available community resources.
• Complying to all Health Department regulations and protocols, including those aimed at protecting personal information.
• Assist the Inspectional Services/Public Health division(s) with duties as assigned.
• These include, but are not limited to, administrative support related to administration of the City’s health codes (Sanitary inspections), program support for the Lynn Council on Aging Senior Center, and assisting the Public Health Director, Coordinator, and Nursing staff with ongoing efforts to mitigate the impacts of COVID-19 and other infectious diseases.
